Most Common Questions About Brain Tumors| Dr. Roopesh Kumar When patients have a brain tumor and when we explain to their family, these are the questions that they commonly ask. Will you open the skull fully? Can this be done using a laser? Will this be done as a laparoscopy? They ask such questions. I would like to provide an explanation through this video. Firstly, there is no concept of opening the skull entirely. Like I just said, after getting access to advanced treatment methods, wherever the tumor is, a small hole is created which we call the window. The bone in that part alone is removed and after removing the tumor, the bone is fixed. There is no necessity to open the skull entirely in most of the situations. We would open half the skull or full skull when in case of a head injury or in case of a severe stroke where the pressure in the brain increases and the entire brain swells up. Only then there would be a necessity to open half the skull. When it comes to a tumor, all tumor surgeries are microscopic surgeries. So there is no necessity to open the entire skull. Wherever the tumor is, a small opening can be made. They ask if we do keyhole surgeries. Surely. A lot of brain surgeries can be done as keyhole surgeries. For some skull base surgeries we would not have to open the skull. A small incision is made near the eye and similar to a keyhole surgery the tumor can be accessed and removed. For skull base surgeries that are done under the brain without accessing the front of the skull. By the method of endoscopy, the tumor can be removed through the nose itself. There are such advancements and treatment options that we have. A lot of patients ask if we use laser. The laser usage is a misconception. For the eyes, there is the option of laser treatment and there are many laser treatment options. Whereas, in the case of the brain, the application of laser is very very less. So in case of brain tumor surgery, we do not use laser. But what they mean is with which advanced technology would we remove the tumor. Most of the time, to remove the tumor from the brain 'suction' is used. 'Suction' is like a vacuum, we use negative suction and only with that negative suction, 80% of the time the brain tumor is removed. Using the suction itself, we gradually cut the tumor and remove it. Bipolar, using electrical current, using short current signals the tissues are broken and removed using suction. They ask us to show the tumor that has been removed. External tumors are removed entirely and shown as a specimen. In the case of the brain, removing the tumor entirely in its shape is very rare. Reason being on the outside is neurons and it cannot be removed like that. Principle of neuro surgery itself is internal decompression. That is, to go inside the tumor, break the tumor cells, using suction for removal is the process of removing brain tumor. So, in the case of tumor specimens, it is not possible to expect a big specimen to be shown in brain tumor surgery. It is cut down using a machine as part of the suction. There is something called TUSA, which is nothing but ultrasound. It is similar to the ultrasound scan done on the stomach. Using ultrasound technology the tumor is broken. Ultrasonic guided aspiration. Using the ultrasonic guidance, the tumor is broken down to smaller bits and we use vacuum suction to remove it. These are all techniques used to remove the tumor. Again these are all advancements in tumor surgery. Importantly, they ask if the surgery can be done through laparoscopy . By using the term 'laparoscopy' they mean if the tumor is removed through a small hole. Laparoscopy is meant for the stomach. The tumors or cysts in the stomach are only removed using laparoscopy. Brain tumors are removed either through endoscopy or microscopy. Those are the terms that you need to understand. So, brain tumors are not removed through laparoscopy and brain tumors are removed either through microscopy or endoscopy.
