SUBAORTIC RING on Transesophageal Echo

This is an interesting case of a 21 year old young male who loved playing football when he was in his early teens but progressively found it difficult to run. By 19 he had stopped playing football. As he was from a remote village in Bengal, the local medical fraternity failed to diagnose the entity. He came to the district hospital only when he started feeling dyspneic even after a brief walk. The diagnosis at the district hospital was that of a aortic stenosis for which he was referred to the city. At our Hospital we found a clear subaortic membrane just below the right coronary cusp extending below the left cusp which was causing a moderate to severe LVOT obstruction. Both the right and left coronary cusps of the aortic valve was so intimately involved with the tissue of the ring that a repair was not possible and he underwent resection of the subaortic ring as well as a AVR.
