Vitrectomy for Submacular haemorrhage | Large submacular haemorrhages can be a huge challenge to manage. Various options such as gas and antivegfs and positioning are done often with the addition of TPA . This video showcases a series of such cases in which we prefer to evacuate the haemorrhage using a posterior smiley shaped retinectomy. We first apply mild diathermy to prepare the smiley shaped incision . Cutter speed is reduced to just 100 cuts a minute and it works like a well controlled scissor in this situation.Once the retinectomy is made we gently insinuate the cutter port into the opening and gently engage the thick haemorrhagic clot by using high vacuum.. The whole haemorrhagic clotted complex gets engaged and is delivered in toto. After this i use air fluid exchange, gentle endolaser to the site and gas injection followed by postoperative positioning.
