How would you like your eggs sir? Solitary Cutaneous Mastocytoma - benign proliferation of mast cells forming a discrete fleshy red to yellow nodule up to 3 cm in size. Mastocytoma is a rare variant of mastocytosis (proliferation of mast cells), accounting for total of 10-20% of cases of cutaneous mastocytosis. They are almost exclusively found in children and are most common on the trunk and wrists. Microscopically, there is a multinodular to diffuse sheet of mast cells in the dermis and occasionally extending into the subcutis. Mast cells in solitary mastocytoma are generally round with a centrally located nuclei (similar to that of a sunny side up eggs) light pink granular cytoplasm. Usually no spindled mast cells. Read on if you need a refresher on mast cells/cutaneous mastocytosis. Mastocytosis: Myeloproliferative disorder, characterized by clonal proliferation of mast cells. These lesions are much more common in children. Can be subdivided into systemic (20%) and limited to the skin (80%). (As an aside, a review of the function of mast cells) Mast cells contain histamine and heparin (anticoagulant) and serve as one of the primary mediators of the allergic response. Once an allergen binds to IgE receptors on the mast cell, histamine, and other inflammatory mediators are released, resulting in vascular dilation and edema; depending on the amount of histamine released, the clinical symptoms may range from mild erythema (redness), pruritis, and flushing attacks, to life-threatening anaphylaxis and hypovolemic shock (liquid/plasma exits vessels and moves into tissue). The diagnosis of skin limited mastocytosis can only be made after exclusion of systemic mastocytosis. There are four forms of cutaneous mastocytosis: 1. Solitary mastocytoma 2.
