This a case of a total plastic rigid pupil with a complete fibrotic adhesion of the posterior surface of the iris to the the anterior capsule , A totally non stretchable pupil with tendency to cheese wire and a floppy iris , use of iris hooks was not considered as the pupil was thick rigid and probably would cheese wire at points of iris hooks and cause a gross distortion of the pupillary shape and to top it up ...A thick fibrotic anterior capsule resisting capsule tear ,and sticky complicated cataract refusing to separate from the capsular bag. This video demonstrates the help of a B hex capsule expansion ring to manage such a complicated case .
