We performed surgery for one of the most complex and strategic tumors of the biliary tract — the Klatskin Tumor (Bismuth Type-4) — using Prof. Dr. Ünal Aydın’s original technique, which has been published in the Annals of Surgical Oncology. In this operation, 11 bile ducts were reconstructed and connected to a newly created jejunal loop from the small intestine. In this video, you are watching the actual operation performed using my technique. We identified the lesion as Bismuth Type-4, and although it was Type-4, not because it was Type-4, we decided to proceed with the surgery and completed it using the Ünal Aydın Technique — bloodless, clean, and perfectly anatomical. The patient had been previously deemed inoperable in other centers. After PTK and ERCP interventions, a cystic lesion appeared in the retro-pancreatic area, suspected to be a second primary. Multiple biopsies failed to give a definitive diagnosis. Our radiologic assessment confirmed it as a cystic formation, not a second tumor, and revealed that the patient had a surgically operable Bismuth Type-4 Klatskin tumor.
