ACIOL Exchange SICS and SHF of 3 Piece IOL

Thanks to G for allowing me to share her surgery. G had previous surgery as a child and then had an AC IOL placed into her eye to correct her aphakia. She had suffered chronic macular oedema and vision was poor at worse than 6/60. After trying everything else she was keen to have the lens removed and replaced as it seemed likely that the lens was rubbing and iritating her iris contributing to the chronic macular oedema. Surgery ewas carried out under local anaesthetic. The AC IOL had to bne removed intact adn this required a large incision. I elected to use the 'SICS' incision as I find this very stable without sutures. Having removed the lens and after pausing about quitting whilst I was ahead (!) I went on to use the needle haptic fixation technique to secure a 3 piece lens behind the iris. It was technically a little more diffiucult as this large wound had the lens haptic opening it resulting in an eye that could not hold firm whilst I tried to push the second needle into the eye. With care the second needle was positioned and the haptics retrieved and fixed. The lens centred well.
