Vibroplasty is an advanced surgical technique used for hearing rehabilitation in patients with middle or inner ear pathologies. Beiletes et al. suggested a classification system for different types of applications of active middle ear implants, or AMEIs, in specific pathological conditions. Type A Vibroplasty This involves coupling an AMEI to an intact ossicular chain in patients with mild to moderate sensorineural hearing loss. The classic indication for this approach includes cases where the ossicular chain is functional but requires amplification. The most favorable attachment points include the stapes head and footplate. Commonly used implants in this scenario include the VSB, MHI, and Carina. Type B Vibroplasty This method is used in cases where the ossicular chain is partially damaged. Here, an AMEI, most commonly the VSB, is coupled to a remnant of the ossicular chain, often the stapes or its footplate. While still considered experimental, research continues to explore additional couplers or passive prostheses with devices such as the Carina and Esteem. Type C Vibroplasty In this technique, the actuator of the AMEI is coupled to one of the middle ear window membranes—typically the round window. Among the currently available devices, the VSB is the only implantable actuator suited for this application. In recent years, round window coupling has gained popularity as an alternative to the classic attachment of the VSB to the long process of the incus. Type D Vibroplasty This is a more advanced approach where the AMEI is directly coupled to the inner ear fluid. In most cases, the oval window is utilized for this procedure. The DACI system was specifically designed for this application, though the VSB, in combination with a conventional stapes piston, can also be used. Each type of vibroplasty plays a crucial role in optimizing hearing restoration based on the patient’s anatomical and pathological conditions.
