Specific Signs & Symptoms: Eye-related Proptosis (bulging eyes) Periorbital edema Gritty or sandy eye sensation Photophobia, excessive tearing Diplopia (double vision) In severe cases: vision loss from optic nerve compression Lid lag (upper eyelid is slow to follow the eye down) Lid retraction (staring appearance) Specific Signs & Symptoms: Skin-related Pretibial myxedema: thickened, waxy, discolored skin (usually over shins) Warm, moist skin due to increased blood flow Specific Signs & Symptoms: Thyroid-related Diffuse goiter (enlarged thyroid gland), often painless but soft Possible bruit over thyroid due to increased vascularity Other Signs & Symptoms Hyperreflexia - overactive or exaggerated muscle reflexes Tachycardia or irregular rhythm Graves Disease - Lab Findings Lab tests: Decreased TSH, Elevated Free T3/T4, Elevated TSI (thyroid-stimulating immunoglobulins) Normal Thyroid: Isthmus Depth: Less than 0.5 cm (approximately) AP Length (Lobe) : Less than 2 cm (approximately) Length (Lobe): 4-7 cm (approximately) Width (lobe) : 2.5 cm (approximately) Ultrasound Findings: Diffuse enlargement of both lobes and isthmus – Often symmetric but can be asymmetric – Volume is significantly increased Heterogeneous and hypoechoic echotexture Nodules: Usually absent in classic Graves'. If present, evaluate for malignancy or toxic nodules Thyroid Inferno: Markedly increased blood flow on color Doppler Called “thyroid inferno” due to intense vascularity One of the hallmark features of active Graves'
