Proof of Identity (Choose one) Valid U.S. Passport or Passport Card Birth Certificate (must be a certified copy, not a photocopy) Permanent Resident Card (Green Card) or other valid immigration documents Federal or State Government-issued ID (such as a U.S. Military ID) Consular Report of Birth Abroad (if born outside the U.S.) 2. Proof of Social Security Number (SSN) (Choose one) Social Security card W-2 form Pay stub with SSN Tax document (e.g., IRS Form 1040) 3. Proof of Michigan Residency (Choose two) Utility bills (electricity, water, gas, etc.) Lease or mortgage agreement Bank statement Tax return Paycheck or government check Michigan voter registration card Official mail from government agencies OR any kind of letter that has your Michigan house address and your name. 4. Proof of Legal Presence in the U.S. (if applicable) Valid U.S. Passport or Passport Card Permanent Resident Card Work Visa Other immigration documents (for non-citizens) 6. Payment Pay the fee for the Michigan ID, which can vary depending on the type (typically $10–$15 for a standard ID). Payment methods include credit/debit cards, checks, or cash. Additional Information If you are under 18, a parent or guardian must be present to sign the application and provide documents. You may need to take a photo for the ID at the SOS office.
