Tissue expanders are used in burn reconstructions quite extensively. Typically you would understand that because if burn is there are a lot of tissues or a lot of areas which are damaged and when you are trying to reconstruct and area which is functionally important or esthetically important. You need to find tissues but because of the lack of normal tissues or the lack of similar kind of tissues, you might be left with very limited option. So what you can do is use of tissue expansion. Tissue expansion is basically a balloon, you put under the skin to expand the skin and as it stretches it recruits new skin and that is how you use that new expanded skin to cover a defect. In burns usually the tissue expansion used is in the scalp because hair bearing area is very unique, it is only present in the head. So when you need the hair bearing area, what you need to do is when there is a burn, that area is removed and you put a tissue expander and a normal skin which has the hair and you expand that and you move it in. So you can do it for the face and the arms and whichever area involved and how gross is the deformity causing functional and esthetic impairment and as there any tissue around where you can use it. Tissue expanders comes in various sizes. So depending on the size of the defect where you want to move is the skin or where you have to out in the expanders. So depending on that there are different sizes and slowly you increase the expansion. Usually when you are doing a tissue expansion, and you overfill, and you need a 1 cm or a 2 cm defect, but you overfill so that the skin goes upto 3 to 4 cm so that when you remove it, and you stich it up, the skin still stays there. But the problem with burns patients is that there are a lot of areas that are already involved. So finding good normal tissues for us to put is a tissue expanders is kind of difficult. Sometimes in critical areas, when you put in a tissue expanders because the skin is already damaged, you still can do that do a tissue expansion but you have to do it more slowly but many at times that might not work as well as normal tissues. So try to put in normal tissues as much as possible but in unavoidable circumstances you can even put a scar tissue and put it slowly and try and get as much as tissue s as you can do it but you have to do in a slow and a stepwise manner. Burn reconstructions are a very extensive and a difficult topic. It is not for everybody and you need to have time and patience and you need to gate a lot of planning to get an optimum result.
