Although many sphenoid wing meningiomas show underlying hyperostosis of the inner table at the site of dural origin, nevertheless those with extensive full thickness infiltration of the skull base by an intradural mass or en plaque extension are a distinct group. The “bony” component may result in proptosis, ophthalmoplegia, reduced visual acuity, and, rarely, other cranial nerve deficits such as trigeminal nerve dysfunction. As the sphenoid bone is the predominant center and origin of these meningiomas, then understanding the anatomy of this bone is important in evaluating the imaging, performing surgical resection, and explaining the extent and extension of these tumors.
