University of Foggia In this video, 43 y/old man non responsive at alfa-blokers drugs. Uroflowmetry and cistoscopy demostrate asymmetric BPH in absence of other cervico-urethral pathologies. Prostate volume 45 cm3. IPSS 25/35. IIEF 22/25. Multiple Urolift device per side are implated, at 11' o'clok, until an adeguate enlargement of prostatic urethra. UROLIFT is an urological surgical device to treat patients with sintomatic benign prostatic hypertrophy BPH. Currently, this technique, is the only one that preserve ejaculatory function in 100% of patients compared to bipolar resection TURP or other that use lasers. In our hands, at mean follow up of 19.35 ± 13.8 6–48 month, 80% of patients were satisfied: Q-max increase: 69.61% p=.008, residual post-void volume reduction of 50.4% p=.03 and reduction of symtoms of 9.41 points 50.43% at IPSS questionnaire p=.0001. This technique is mainly offered to patients interested to maintaining ejaculation function and especially to young people suffering from BPH that are not responsive to farmacological treatments. Attualmente, questa rappresenta l'unica tecnica che garantisce la preservazione dell'eiaculazione nel 100% dei pazienti rispetto alle altre tecniche come la tradizionale resezione di prostata TURP o le altre che usano i lasers. Nella nostra esperienza, ad un follow up medio di 20 mesi circa, l'80% dei pazienti è soddisfatto: abbiamo avuto un aumento significativo p=.008 del picco massimo urinario del 70%, una riduzione significativa p=.03 del volume residuo del 50% e una riduzione significativa dei sintomi del 50% p=.0001 al questionario soggettivo IPSS somministrato ai follow up. Questa tecnica è riservata ai pazienti interessati a conservare l'eiaculazione e soprattutto a pazienti giovani che soffrono di ipertrofia prostatica e non responsivi ai trattamenti farmacologici.
