In this example, the surgeon has identified an accessory left pudendal artery and is planning to preserve this. The prostate is dissected fully prior to the apical dissection. At this point, the surgeon puts traction on the prostate proximally and cuts through the apex with the robotic Endoshears. There is some bleeding, and this is controlled to some extent with monopolar energy. But there is aAfterward, the surgeon meticulously dissects along this structure, using biopolar cautery to control the DVC as it is cut across. Once dissection of the DVC is completed, the urethra is better appreciated, and this is incised sharply. Once the anastomosis is completed, there is no bleeding seen from the DVC, and so no specific suture is placed. Rather, the surgeon uses an anterior suspension stitch to reapproximate tissue over this bare area as a final step.
