We begin with the origins in the vestibular nuclei located in the brainstem, which receive input from the inner ear's vestibular apparatus. These sensory signals provide continuous head position and motion updates, allowing the vestibulospinal system to generate rapid postural adjustments. We then examine the two main components of the vestibulospinal system. The lateral vestibulospinal tract descends ipsilaterally through the spinal cord to influence extensor muscles of the trunk and proximal limbs, playing a significant role in upright posture and balance. In contrast, the medial vestibulospinal tract projects bilaterally to cervical and upper thoracic spinal cord segments, coordinating head and neck movements in response to changes in body position. The lecture details their anatomic pathways, synaptic targets in the ventral horn, and their relationship to other descending tracts for integrated motor control. Finally, we connect these pathways to their functional significance, explaining how they work with visual and proprioceptive inputs to stabilize the body during movement. We discuss the effects of lesions to these tracts, including impaired balance and abnormal head positioning, and reinforce their role in reflexive postural control.
