The long thoracic nerve transmits nerve signals from cervical spinal nerves C5-C7 It travels between clavicle & first rib through axilla along chest wall to supply motor innervation to the serratus anterior muscle. Mechanism of Injury: Sudden upper extremity traction. Shoulder depression with contralateral cervical lateral flexion. Prolonged compression (backpacker's palsy) Symptoms: Pain in shoulder, scapula and/or neck. Usually worsened with activity that causes traction on nerve. Decreased shoulder flexion strength. Scapular winging at inferior border, worsened with shoulder flexion. Recovery of nerve function is more likely with a mild injury and a shorter duration of compression. Recovery is faster if activities that exacerbate injury are decreased or ceased.
