CT Scan – Paranasal Sinuses (Plain): Key Points 🧠 Purpose of the Scan To evaluate the anatomy and any abnormalities in the paranasal sinuses – maxillary, frontal, ethmoid, and sphenoid sinuses. To assess for causes of: Chronic or recurrent sinusitis Nasal obstruction Facial pain/headache Preoperative planning (e.g., for FESS – Functional Endoscopic Sinus Surgery) 🔍 What the Scan Shows Sinus Opacification Normal sinuses appear air-filled (black on CT). Opacification (white/grey areas) indicates fluid, mucosal thickening, or mass. Mucosal Thickening Often due to chronic inflammation or allergy. Can be localized or diffuse. Air-fluid Levels Suggest acute sinusitis or secondary infection. Seen as a straight horizontal level in the sinus. Polyp or Mass Lesions Soft-tissue density in sinuses or nasal cavity. May need further evaluation (contrast CT/MRI or biopsy). Deviated Nasal Septum (DNS) Common anatomical variation. Can contribute to sinus drainage issues or nasal obstruction. Osteomeatal Complex (OMC) Status Critical drainage area for anterior sinuses. Blockage here often causes maxillary and frontal sinus disease. Bony Erosions or Remodeling Seen in aggressive infections, fungal sinusitis, or tumors. Anatomic Variants E.g., concha bullosa, Haller cells, Onodi cells – may impact drainage and surgical approach. Frontal Recess / Sphenoethmoidal Recess Evaluation Important for drainage of frontal and sphenoid sinuses respectively. ⚠️ Why It Matters Guides diagnosis and treatment (medical or surgical). Essential before any sinus or skull base surgery. Helps rule out red flags like invasive fungal sinusitis or neoplastic lesions.
