A Bankart lesion is a tear of the shoulder labrum and capsule which often occurs when the shoulder is dislocated. Surgery is usually needed and after surgery, rehabilitation is typically broken down into 3 phases: the immediate post operative phase, the intermediate post-operative phase and the advanced phase/return to work and sports phase. In this video, I demonstrate the exercises that I give my patients who are in the third phase, the advanced post-operative phase. In this phase of rehab we want to normalize strength and range of motion. We start to gently stretch into movements that were previously restricted. The movement restrictions that were previously in place are done because the tissue has healed enough to tolerate some stress. However, we still need to be a bit careful as the repaired tissue is not yet totally healed. Gentle stretching is fine, but aggressive movements that stress the tissues in the front of the shoulder still need to be avoided (throwing for example). The goals of this phase are to normalize strength and range of motion, gradually increase stress to the repaired part of the shoulder (the anterior capsule), and gradually return to all daily and work activities. We need to avoid applying stress to the anterior capsule too aggressively. Aggressive stretches to the pectoral muscles should be avoided. In addition, weight lifting maneuvers such as the pec-deck, chest flys, lat pull-downs, dips, throwing, swimming should be avoided until after about the 16th post op week.
