Glue ear is also also known as secretary otitis media, it is also called as serious otitis media or otitis media with effusion. It is a common conditions scene in clinical practice, more of this condition is seen in children. The commonest age group by 2 and 5 years of age, however it can occur in any age group. In children because there they are a bottle fed infants their prone to recurrent upper respiratory tract infection, slightly older children are also very prone to recurrent upper respiratory tract infection. As they keep going to the school and pick up lot of infection from the daycare centres and immunity system will be just developing, because of the short and wide eustachian tube which connects the back of the nose to the ear, children are very prone to develop middle ear infection followed by glue ear. If the acute otitis media that is middle ear infection is not treated appropriately with full course of antibiotics and decongestants, child is susceptible to develop a secretary otitis media. In glue ear the fluid which is present in the Middle ear is thick and sticky but it is not a infective fluid, therefore the child will not have symptoms of severe ear pain a fever which happens in acute otitis media. It is a painless condition and it can go easily unnoticed by parents or the children. The most common presenting symptom being hearing loss and in attentiveness. The child may not respond, may be overactive and irritable, restless. So this condition needs to be appropriately diagnosed at a correct stage. Management of glue ear consists of Investigation and treatment. Treatment of glue ear consists of initial medical line of Management and within 3 months if it does not subside on its own we need to do a minor procedures called myringotomy and grommet insersion. However sometimes after the grommet falls off, the child can have recurrent cough symptoms in the form of recurrent glue ear, in such cases we need to resort to putting what is called as a T tube this is somewhat similar to the grommet but it is a long lasting. This T tube or the grommet provides sufficient and continuous air to the middle ear there by avoiding conductive deafness.
