This video shows the removal of an opacified multifocal Lentis Mplus lens. A percentage of these lenses became opacified over time. It is thought to be due to a phosphate-containing cleaning agent used in the manufacturing process, and the lens was recalled from the market. This patient had the Oculentis Mplus Lens implanted 9 years ago and started to notice gradual opacification over the last 3–4 years. The vision dropped to 6/12, and he was quite troubled with foggy vision. Slit lamp examination showed significant opacification of the whole lens. In addition, the patient had severe zonular weakness. The video shows the steps of the surgery and techniques to deal with zonular weakness. The main tips are: Minimizing any stress on the zonules by using counter-traction forces while explanting the lens. Repeated use of viscoelastic throughout the surgery. In such situations, you may achieve better long-term stability of the secondary implant by using a 3-piece lens with optic capture.
