When dealing with major trauma or major deformities. It could be congenital or because of trauma or burns. What happens is you need to reconstruct that part or that area which is missing or which is scarred. When we are doing reconstruction, the first and the foremost thing of reconstruction is that you have to replace a like tissue by a like tissue, what it means is that you have to replace it with similar looking tissue if no the exact same tissue. Every tissue in the body are different there are certain differences which are innate to that area and if you want to reconstruct tissues on the face, you would prefer tissues around it and if you want instead of taking tissues from the leg or the thigh which are totally a different match. So what happens is when you are looking at tissues in the vicinity, there is a shortage. So how do you overcome shortage? It can be overcome when you bring in tissues from somewhere else like from the thighs or the legs, but it is not the proper match of the texture, colour, hair growth, structures within the whole flap. So you will use tissues that surround that area like from the face you can use tissues from the neck. On the neck you can use tissues from the neck and the shoulders and what happen is because it is in the closer vicinity there is a proper texture and colour match, So for that what because you have limited skin you need to expand the skin or make more skin is that is where you use tissue expanders. tissue expanders is basically a balloon which is put underneath your skin and you slowly inject water or saline to blow up the balloon. So as the balloon increases the size, it stretches in size and form new skin. It is basically the same thing that happens in pregnancy ,the tummy is expanding because the baby is growing inside and as the baby is growing, the baby also grows and that so also how the tummy expands. So once the baby comes, the skin goes back. So that is the elastic property of the skin and also you are making new skin because of the stress of the stretch. So what you are doing is you are bringing more normal tissue that is innate to that area. So this whole process is a staged procedure. The first stage is where you put the tissue expander, you close it. After 4 to 5 days you fill it with water and every second day you fill it with water and as you fill in water, the tissue starts expanding and at the end of about a month, you might have achieved the amount of expansion required and the second stage, you take the excess skin, you pull it across. So that is how the basic principle of tissue expander works.
