PATH Street Outreach Video Series - Episode 3: Final Encounter (Connecting to Housing)

PATH (Projects for Assistance in Transition from Homelessness) provides assistance to people who are experiencing homelessness and who have a mental health condition. PATH is funded through the Substance Abuse and Mental Health Services Administration and supports street outreach and case management services to help connect people to mental health services, primary health care, income support, and housing. HOPES Center's street outreach program uses the PATH Street Outreach Video Series as part of its orientation for new outreach team members. Although there are some differences between what is shown in the video (Venice Beach, California during the day vs. Racine, Wisconsin at night), but the principles are the same and the video is a good representation of street outreach services and techniques.
