Precipitating factors of hypothyroidism into myxedema coma are infections, exacerbations of comorbidities, trauma, and other stress inducers. Patients can have a nonspecific presentation initially. Address fluid balance and electrolyte replacement, vasopressors, antibiotics, in addition adequate ventilation. Adrenal insufficiency often overlaps with myxedema coma and stress dose hydrocortisone is coadministered with suspicion. While all thats going on, Low intracellular T3 is the underlying pathology. T4 is converted to T3; The American Thyroid Association recommends load with 200 - 400 mg IV, followed by 1.6 mcg/kg enteral and multipled by .75 if given IV. Liothyronine, isn’t available IV in the US and comes enteral. The ATA guidelines also recommend liothyronine 5 to 20 mcg, followed by 2.5 to 10 mcg every 8 hours.Consider lower end of dosing range for the brittle and those with extensive cardiac hx.
