The blockages in the tube, it can be a proximal blockage, mid segment blockage or distal blockage. Most of the blockage are due to infection. Post infection, there can be blockages in the tube. Sometimes there can be water collection in the tube, sometimes there can be pus collection in the tubes. We call it as a hydrosalpinx or a pyosalpinx when there is pus inside the tube. So we have different options of the different types of a block. When there is a sever block with a pus it is called as a hydrosalpinx. In most of the cases, we don’t try to open up the tubes because when we try to open up, it may not be patent post-surgery. There can be active infection when there is fluid or a pus collection. So in those cases, the fluid is trying to go inside the uterus, that is not good environment for the embryo to implant into the uterine wall. When there is pus in the tube or a fluid in the tube, in most of the cases, they are not able to conceive, we ask them to removal of the tubes and go for an IVF baby. So most of the infection leads to an impairment of the implantation. Proximal tube blocks, in most of the case it can be due to a spam of the tubes and sometimes the infection leads to a distal block. Proximal tubal blocks are good kind of blocks and in post-surgery most of the cases, they are able to conceive. In Proximal tube blocks, we put in a camera inside the uterus called as the hysteroscopy and when there is a definite block in the laparoscopy when we check from the abdomen, we put in an instrument called the cannulation wire. So we put the cannulation wire from the hysteroscopy into the tube to inside the uterus and it will release the block when there is a block in the proximal tubes only. When there is block away from the Proximal tubal, we don’t try to correct with the hysteroscopy cannulation. Sometimes there is block in the distal tubes. The fimbria is what we call as the distal end. It gets blocked or gets attached to the surrounding tissue. So that time we can try to release the distal end of the blocks. Sometimes when we try to release the distal end of the blocks. Many times when we release the distal end of the blocks, there is a chance for tubal pregnancy, we call it as an ectopic pregnancy. So especially when we treat for the tubal blocks, we tell the chances for a tubal pregnancy in future. If they conceive they have to meet the doctor immediately to know whether the tube is having a pregnancy or the uterus is having the pregnancy to rule out ectopic pregnancy.
