Endless mode only ends when you die. Random specimens can chase at any time with a new one being added to the possible chase pool within every 100 rooms. You can be chased by up to a maximum of 5 specimens at once and sometimes even multiple of the same specimen, It all comes down to RNG and your own skill. Specimen 1 and Specimen 2 are present in the chase pool by default. Oh and there's also Steam leaderboards for bragging rights. This is my 3rd run in total. Now with Dollhouse enemies.
