ENT Manifestations of Migraine 8 - Ear Fullness and Atypical Facial Pain

Many patients search with specialists looking for a solution to chronic symptoms such as ear fullness and atypical facial pain. Some come with persistent symptoms after tooth removal fail to resolve symptoms. This patient has no headache but had chronic symptoms that responded well to low dose nortriptyline. He is an example of how useful low dose nortriptyline can be in the therapeutic armamentarium of the otolaryngologist. He did not have eustachian tube or sinus problems that would have most directly answered his temporally related symptoms, and his attempts to address symptoms with dentists and allergists were not fruitful. This patient may have symptoms attributable to neural hypersensitivity suggestive of migraine but he has no headache, headache history and no exacerbation of symptoms with common migraine triggers. Nonetheless we were able to provide some relief because of our awareness of atypical presentations in ENT like migraine.
