Uncal herniation occurs when rising intracranial pressure causes portions of the brain to flow from one intracranial compartment to another; this is a life-threatening neurological emergency and indicates the failure of all adaptive mechanisms for intracranial compliance. Kernohan notch phenomenon is an imaging finding resulting from extensive midline shift due to mass effect, resulting in the indentation in the contralateral cerebral crus by the tentorium cerebelli. This has also been referred to as Kernohan-Woltman notch phenomenon and false localizing sign. Duret hemorrhage is a small hemorrhage (or multiple hemorrhages) seen in the medulla or pons of patients who are rapidly developing intracranial herniating. Parinaud's syndrome is an inability to move the eyes up and down. It is caused by compression of the vertical gaze center at the rostral interstitial nucleus of medial longitudinal fasciculus (riMLF).
