What is a Chronic Condition?

Chronic diseases are long-term health conditions that can have a significant impact on a person's quality of life. In order for patients to qualify for Chronic Care Management, the Centers for Medicare and Medicaid Services (CMS) require patients to have two or more chronic conditions.
