In this video, I present the Aortic Pulsatility Index (API) and its potential in optimizing the management of cardiogenic shock. We delve into the current management of cardiogenic shock, highlighting the limitations of existing hemodynamic parameters like Cardiac Power Output (CPO) and Pulmonary Artery Pulsatility Index (PAPi). Enter the API—a newly explored metric that aims to simultaneously represent cardiac function and filling pressures. We explore the calculation of API and discuss the challenges. Additionally, we analyze the suggested cutoff values for API and its predictive ability for patient outcomes in cardiogenic shock and decompensated heart failure. While API shows promise, its validation in real-world scenarios and prospective studies is limited, and its performance in patients on vasopressors or with acute myocardial infarction remains unknown.
