Median Arcuate Ligament Syndrome, or MALS, is a rare but serious condition caused by compression of the celiac artery and the celiac ganglion nerve plexus by the median arcuate ligament. This compression leads to severe, debilitating abdominal pain, especially after eating, and can result in significant weight loss if left untreated. In this video, Dr. Danny Shouhed, a minimally invasive gastrointestinal surgeon, explains the anatomy behind MALS, the hallmark symptoms, and why the condition is so challenging to diagnose. He discusses how MALS pain differs from other GI conditions, the associated disorders often seen in patients (including POTS and Ehlers-Danlos syndrome), and why listening carefully to each patient’s story is a critical part of making the diagnosis. Dr. Shouhed also reviews the tests used to confirm MALS, including endoscopy, CT scan with contrast, duplex ultrasound, gastric emptying studies, and the importance of the celiac ganglion nerve block in determining whether surgery will provide relief. He then outlines his surgical approach to treating MALS. Unlike traditional open surgery, Dr. Shouhed performs this operation robotically through small incisions, with excellent outcomes. In his first 100 patients, 83% experienced significant improvement after surgery, with no mortalities and a conversion rate to open surgery of less than 2%.
