The white Morgagnian cataract has a thin fragile capsule, weak zonules and no epinuclear shell to hold the PC back during phacoemulsification, all of which lead to an increased risk of PCR. In this video, after rhexis, I prolapse out the nucleus into the AC and implant the IOL into the capsular bag. The cataract is then emulsified using the IOL as a scaffold to prevent posterior capsular rupture (PCR). This technique follows the IOL scaffold technique that has been described by us earlier (IOL scaffold for PCR. JRS 2012) and the specific application of the IOL scaffold technique for Morgagnian cataracts in the absence of PCR as shown in this video has been proposed by Dr. Rohit Om Parkash (Parkash RO et al. Intraocular lens scaffold technique to prevent posterior capsule rupture in cases of Morgagnian cataract. JCRS 2017). This video also demonstrates the use of the beautiful B-HEX ring which is an elegant and effective pupil expander device that is also easy to implant and explant
