The three main ways of fixating a three-piece IOL to the sclera without using sutures: the Yamane technique, the Scharioth technique, and the Agarwal technique. These are all good options but they are each a little different. The Yamane technique uses cautery to create a flange on the end of the haptic so that it gets wedged into the scleral wall, the Scharioth technique places the haptic within a small scleral tunnel created by a needle, and the Agarwal glued IOL technique improves on the Scharioth technique by creating a scleral flap and then using tissue glue to adhere the flap and the haptic within the tunnel. This video shows our guest surgeon using the Scarioth technique to successfully complete a very challenging case.
