Dickon Hayne, MBBS, MD, FRCS, FRACS, The University of Western Australia, Perth, Australia, comments on whether all patients with localized bladder cancer, especially muscle-invasive bladder cancer (MIBC), need a cystectomy. Whilst neoadjuvant chemotherapy and surgery is the standard of care in MIBC, trimodal therapy consisting of chemotherapy, radiotherapy and transurethral resection of a bladder tumour (TURBT), as well as quadrimodal therapy with the addition of immunotherapy is gaining traction. Bladder preservation should be the ultimate aim whenever possible.
