In this pocket tips, we show how to make the antibiotics dilution commonly used in the treatment of endophthalmitis. We know that endophthalmitis is a serious and devastating condition, and regardless of whether pars plana vitrectomy will be performed or not, we must collect material for culture and inject broad-spectrum antibiotics as soon as possible. Vancomycin to cover gram positive, and Ceftazidime to cover gram negative agents is one of the most used regimens for treatment. To prepare Vancomycin, you must first take the 500mg antibiotic ampoule, a sterile water for injection, a 10 ml syringe and an aspiration needle. Aspirate 10ml of the water for injection, and dilute the content of the Vancomycin ampoule, shaking the vial to dilute the medication well. This way you get a concentration of 50mg/ml. If you need to do a subconjunctival associated injection, use this concentration and inject 0.5ml or the equivalent of 25mg. For intravitreal injection, aspirate 1 ml of this concentration into a different syringe and dilute with a further 1 ml of water for injection. This way you get a vancomycin concentration of 2.5mg/0.1ml. Aspirate 0.1ml and inject into the vitreous cavity. To prepare ceftazidime, take a 1g ampoule of antibiotic. Aspirate 5 ml of water for injection and dilute the ceftazidime. Shake the bottle and in this way you will get a concentration of 200mg/ml. If you need to make a subconjunctival injection, aspirate 0.5ml and inject at this concentration the equivalent of 100mg. For intravitreal injection, take a new syringe and aspirate 1 ml from this vial of ceftazidime, then aspirate another 7 ml of water for injection. This way you get a ceftazidime concentration of 2.5mg/0.1ml. Aspirate 0.1ml and inject into the vitreous cavity.
