Recorded on 3/13/2021 51-year-old male with T3N0 distal rectal adenocarcinoma, microsatellite stable. Panelists would recommend a total neoadjuvant treatment (TNT) approach with chemoradiation first, followed by chemotherapy, followed by surgery in an attempt to avoid a permanent colostomy. With involvement of the anal canal, the inguinal lymph nodes would be at risk as well and panelists would recommend prophylactically targeting these nodes with radiation. Given the distal location of the tumor, there is increased likelihood of needing an APR, but this could be avoided with good response to TNT. If good response to TNT, could use a watch and wait approach without surgery.
