Phacoemulsification:@0:10 Most ideal surgery but expensive, Suture less (because of the size of the incision and the multiplanar incision). Incision: Less than 3 cm in size ( very small incision that the lens cant drop out) Valvular/ multiplanar incision( Incisions are made in three different planes, hence creating a valve) Self – sealing ( Valve is self sealing and automatically seals incision) Scleral/ corneal tunnel incision( Tunnel – has a roof, floor and walls) Lens emulsified by a metallic tip with ultrasonic frequency - 40,000/s (tip breaks the lens with 40000s frequency) Foldable IOL (Silicone/ Acrylic IOL is used) (3mm incision can allow larger 6mm IOL, cause the lens are foldable) Less time consuming, 6/6 vision restored (compared to finger counting at 1 meter in ICCE & 6/18 in ECCE) Just a quick run through the steps in the photo emulsification. We break the lens by phacoemulsification of ultrasonic frequency of 40000/s and finally place a foldable IOL. Phacoemulsification VS SICS@3:58 SCIS more than 6mm incision, that’s the maximum incision that can stay without sutures…more than that 6.1 ,2 would need sutures. Otherwise it will start leaking. SCIS incision is also done in three planes like phacoemulsification. Unlike phaco, SCIS is done manually by which it becomes economical. We take two instruments in two hands and break the nucleus with manual force. IN SCIS, we can use a PMMA IOL cause now that the incision is big compared to phacoemulsification which is just 3mm. And because of PMMA IOL further cost is reduced. Both of them are sutureless and self sealing. So at the end of the surgery SCIS can restore 6/6 vision just like phacoemulsification. And there is no much astigmatism in both the surgeries and hence SCIS is more cost effective. In India most of the cataract surgeries performed are SCIS.
