This video demonstrates a laparoscopic appendectomy performed in a 9-year-old girl diagnosed with plastron appendicitis. The patient presented with 4 days of abdominal pain and vomiting. On physical examination, a firm mass with guarding and rebound tenderness was palpated in the right lower quadrant. Laboratory findings revealed elevated inflammatory markers. A 5 mm camera port was introduced through the umbilicus, and two working ports were inserted laterally. Dense inflammatory adhesions and a phlegmon were identified in the right lower quadrant. Careful blunt and sharp dissection was performed to isolate the appendix from the plastron. The appendix was dissected to its base, ligated, and excised. The abscess cavity and the peritoneal space were irrigated and aspirated thoroughly. A Jackson-Pratt drain was placed, extending to the abscess site and the pouch of Douglas. This video may serve as a visual guide for managing complicated appendicitis with plastron formation in pediatric patients via a minimally invasive approach.
