A 35-year-old woman says that her eyelids have always looked full. In recent years, she has experienced strong fluid retention that makes her uncomfortable. She says she has a lot of extra skin in her eyes which leaves her looking tired and unfriendly. She asks if she would be a suitable candidate for blepharoplasty. Dr. Amiya Prasad, an oculofacial plastic surgeon, explains that it is unusual to have a significant amount of swelling around the eyes which developed into a real pressure healing. He emphasizes about a condition called thyroid-immune orbitopathy or referred to as Graves' Disease. There are many people whose first symptoms of thyroid eye disease include swelling, congestion and variability. Their eyes swell and have double vision. Other things that can cause these types of symptoms include sinus problems and allergies. He recommends that she first see a medical doctor, an endocrinologist, or an ophthalmologist and even consider getting some imaging studies so that she can rule out these as causes. If everything else is ruled out, Dr. Prasad recommends an upper eyelid blepharoplasty and lower eyelid blepharoplasty. Upper eyelid blepharoplasty is a procedure where excess skin over the eyelid as well as soft tissue and fat is sculpted and reduced so that the eyes appear more open or awake. For the lower eyelids, there can be reorientation of fat pockets, the reduction of skin, and tightening of the support so that they don't look puffy.
