In stage 3 ovarian cancer, cancer spreads to the peritoneum (the lining from pelvis to diaphragm). Measuring this spread helps doctors decide between surgery and chemotherapy and predict prognosis. Why is Measuring Peritoneal Metastasis So Important? Studies show the biggest factor influencing survival is how much cancer remains inside the peritoneum after treatment. Accurate measurement allows doctors to track disease, plan surgery, and improve results. What is PCI Score (Peritoneal Carcinomatosis Index)? The PCI Score, developed by Dr. Paul Sugarbaker, divides the abdomen into 13 regions. Each is scored 0–3 depending on tumor size. The total (0–39) provides an objective measure of peritoneal metastasis. How Do CT Scan and MRI Help in PCI Assessment? A thin-slice CT scan, sometimes MRI, helps estimate PCI. Key tricky areas include the small intestine, liver surface, omentum, and diffuse peritoneal disease. If high disease burden is detected, doctors may give chemotherapy first before surgery. Limitations of Imaging in Ovarian Cancer CT and MRI often miss tumors smaller than 5 mm. That’s why staging laparoscopy is vital—it allows direct visualization of the abdomen to detect small nodules and calculate PCI more precisely.
