Prof. Ünal Aydın’s Gentle Dry-Field Dissection in Retroperitoneal Lymphadenectomy This case features a 49-year-old female patient with a history of ovarian serous carcinoma who had previously undergone total hysterectomy and peritonectomy, followed by adjuvant chemotherapy. During follow-up, recurrent para-aortic and iliac lymph nodes were detected. Due to their proximity to major vascular structures, the ureter, and autonomic nerves, surgical intervention had been considered high risk and was deferred by other centers. The patient was referred to our clinic for further evaluation and was reassessed within the framework of an organ-preserving, micro-anatomy–based surgical philosophy. Preoperative diffusion-weighted MRI provided detailed visualization of the recurrent nodes, enabling precise surgical planning. The operation was performed through a midline incision. Mobilization of the left colon from the retroperitoneum allowed full exposure of the para-aortic and left iliac regions. Dissection was carried out meticulously along the internal, external, and posterior iliac chains, with preservation of the bladder nerves, ureter, and surrounding microvascular structures. Through synchronized bipolar energy and fine microdissection, the lymph nodes were excised en bloc under completely dry and anatomically distinct conditions. The key advantage of this technique lies in adapting retroperitoneal microvascular dissection principles—originally developed in organ transplantation—to gynecologic oncology. Maintaining a dry, bloodless field facilitated maximal precision, safeguarding both neural and vascular continuity throughout the operation. The patient experienced an uneventful recovery and was discharged within 48 hours postoperatively. This case underscores the anatomical clarity and safety achieved by Prof. Ünal Aydın’s dry-field microdissection technique in retroperitoneal lymph node surgery.
