As the new financial year begins, prevention and self-management remain central to NHS priorities across primary care. This session focuses on practical, proactive ways to identify prevention cohorts and support patients to manage their health more effectively. It will explore how taking a more structured and intentional approach combining data, clear workflows, and proactive outreach can help practices identify patients earlier, intervene at the right time, and reduce the risk of issues escalating into more acute need. By catching problems before they escalate, practices can support preventative care, reduce avoidable demand on GPs, and ensure patients are seen at the most appropriate point in their journey. The session will also look at how clear, consistent patient communication and trusted self-care resources can improve patient understanding, confidence, and engagement, helping patients play a more active role in managing their own health. We will be joined by Dr Tom Micklewright from @ORCHAHealth, who will share evidence on the role of patient-facing apps in supporting self-management as part of a wider, balanced approach.
