Third esophageal self-expandable metal stent

62 years old male. Laryngectomy few years ago because of laryngeal cancer with definitive tracheostomy. Diagnosed of unresectable esophageal epidermoid carcinoma initially treated with chemotherapy and radiotherapy and in a second time with palliative partially covered self-expandable metal stent (SEMS) placement. Few months later he complains of worsening of dysphagia with obstruction at both ends of the stent by ingrown, treated with a new longer SEMS. Again ingrown develops at both ends with almost absolute dysphagia and signs of tracheoesophageal fistula. New SEMS was successfully inserted, longer than the previous two SEMS, having little room for placement since the obstruction is about 3 cm of the upper esophageal sphincter.
