Sri Shankara Cancer Hospital & Research Centre, Bengaluru hosted The International Head & Neck Oncology Conference (COMPLICON) in August 2024. Various guests from all over India and the world were invited to share their expertise and experience on different head and neck cancers. Speaker: Dr. Akshay Kudpaje Topic: Management of Postoperative Trismus In this video, Dr. Akshay talks about: a. An Overview of Trismus: Trismus, or restricted mouth opening, is a significant morbidity in head and neck cancer patients, impacting essential daily functions like eating, speaking, and oral hygiene. It also hinders follow-up examinations and denture placement, ultimately affecting the patient's quality of life. b. Etiology of Trismus in Head and Neck Cancer: Trismus in this population arises from various factors including pre-existing conditions like submucous fibrosis, direct extension of cancer into masticator spaces, and post-treatment effects of surgery, radiation, and chemotherapy, all leading to fibrosis and muscle dysfunction. c. Preventive Strategies and Early Intervention for Trismus Management: Prevention, through appropriate surgical reconstruction and early initiation of mouth-opening exercises, is paramount in managing trismus. In cases with submucous fibrosis, addressing it during surgery and combining intralesional triamcinolone injections with exercise therapy can show improvement. d. Non-Surgical Management of Trismus: Non-surgical management primarily involves consistent and regular mouth-opening exercises using tools like wooden spatulas, heisters. Pharmacological interventions like pentoxifylline have limited evidence, while botulinum toxin and hyaluronidase are being explored. e. Surgical Management of Trismus: Surgical intervention for trismus includes releasing fibrous bands, performing myotomies, and lining the area with skin or mucosal flaps. Coronoidectomy, particularly for mouth opening less than 20mm after failed physiotherapy, has shown to improve long-term outcomes. f. Reconstruction Considerations to Minimize Trismus in Head and Neck Defects: Choosing appropriate reconstructive flaps, especially free flaps for large composite defects, is crucial to minimize trismus. Ensuring adequate lining and addressing potential devascularization of inner skin paddles in regional flaps are vital for long-term mouth opening and function.
