Presenting the paper is dr. Ruth Lumbuun Tuberculous spondylitis is one of extrapulmonary tuberculosis and associated with severe functional impairment. This video represents twenty-one years old male with tuberculosis spondylitis, underwent decompression laminectomy, pedicle screw fixation, and ninth thoracal vertebrectomy by neurosurgeons, followed by corporal defect reconstruction using free fibula flap. One 2.5 cm-segment of fibula harvested from left leg in prone position without skin paddle, was inserted with press-fit method. Anastomosis was done to artery and vein from intercostal artery perforator, size 1 millimeter both. Subjectively, the pain was decreased, motoric strength of both legs are improved, patient is able to sit, hypesthesia is improving, there is positive Doppler ultrasound and bony union noted on the CT scan Free fibula flap reconstruction in spine defect, although not regularly performed, is effective, save, and valuable. This is considered as the first free vascularized fibula flap for vertebral body defect reconstruction in Indonesia. Copyright of dr. Ruth Lumbuun
