There was a time when anterior chamber phakic IOLs were popular for the refractive treatment of high myopia. The challenge with these anterior chamber phakic IOLs was that most required larger incisions since they were made of PMMA which is a rigid plastic and they caused corneal endothelial cell loss. There were anterior chamber phakic IOLs that were clipped/secured to the iris (https://cataractcoach.com/2023/04/03/...) and other designs like shown in today's video were angle supported. Even Alcon developed the Cachet lens which was a foldable anterior chamber phakic IOL made from a hydrophobic acrylic material. While posterior chamber phakic IOLs (like the Staar Visian Evo ICL) are considered safer, they too (like all phakic IOLs, both anterior and posterior chamber) have an associated corneal endothelial cell loss.
