There are many causes of upper airway obstruction (UAO), and obstruction can occur at different levels of the upper airway, UAO can be acute or chronic, infective or non-infective, congenital or acquired. We will discuss the common causes of acute upper airway obstruction including obstruction at the supra-glottic level (quinsy, retropharyngeal abscess, epiglottitis) and obstruction at the sub-glottic level (croup, laryngomalacia, tracheomalacia, mediastinal mass), and some causes that can be either above or below the vocal cords (such as a foreign body). We will show how to use clinical signs to distinguish supra-glottic and sub-glottic obstruction, and discuss how to interpret a lateral neck x-ray. Upper airway obstruction is a common emergency in paediatrics, so paediatricians need to know how to manage this.
