Stage 2 means the tumor has extended from the body of the uterus into the cervix, but hasn’t spread to lymph nodes or distant organs. We confirm this through clinical exams, biopsy, and imaging like CT, MRI, or PET scans. If cervical involvement is evident, we classify it as Stage 2. What is the first step in treatment? We usually start with surgery. The standard approach is radical hysterectomy which means removal of the uterus, cervix, fallopian tubes, ovaries, and nearby tissues. Unlike Stage 1, Stage 2 requires wider margins to ensure complete cancer removal. That’s why we operate carefully around vital structures like the ureters, bladder, and rectum. Why more extensive surgery? The cervix is located deeper in the pelvis and close to important organs. A simple hysterectomy isn’t enough. In radical hysterectomy, we shift the ureters, protect nerves, and ensure clear margins which reduces recurrence risk and improves disease control. Is radiation therapy needed after surgery? Yes. Most Stage 2 patients receive radiation after surgery to lower the risk of local recurrence. Even with clean margins, microscopic disease can remain. The radiation plan is customized based on the final pathology. Do all patients need chemotherapy? Not always. We decide based on tumor grade, lymphovascular invasion, cancer type, and the patient’s overall health. High risk features may call for combined chemotherapy and radiation. If a patient isn’t fit for surgery, we may begin with radiation and reassess before adding chemotherapy. What if surgery isn’t possible? If the patient is unfit for anesthesia, radiation can be the primary treatment.
