Tummy Tuck: Fleur De Lis Incision | Aesthetic Surgical Images One of the most common procedures in cosmetic surgery is the abdominoplasty which we always call the tummy tuck. In the tummy tuck operation, we generally make an incision in the lower abdomen from one hip to the other and lift that skin and fat off the muscle there, often times tighten up the muscle and then pull the skin and fat layer down as snug as we can, tightening up the abdominal skin. Today, we're seeing more and more patient's who've undergone massive weight loss. Massive weight loss is weight loss in the 70 to 100 pound range and when that happens there's often more skin than we can take off with a conventional abdominoplasty. Those patient's they might also require an incision that goes from their rib cage in the midline down to their pubic bone to remove skin in this direction. This operation is called the Fleur-de-lis abdominoplasty. Fleur-de-lis is a French word, that's Fleur-de-lis. It translates into the flower of the Lilly and the reason for that translation is when we take out the skin, the skin is taken off vertically in this direction and horizontally in this direction and it has the appearance of a Lilly, and so that's where the word comes from. It's a very helpful operation when we need it, but it's more extensive and requires more postoperative recovery. We worry more about the juncture where the vertical and the horizontal components come together because those corners have less blood supply than they would normally have and we have greater chance for some wound healing problems afterwards. It's very important to take all of this into consideration, not only in the selection of the patient, but in the performance of the surgery. Sometimes patient's will come to us with a scar on their abdomen that requires this operation to get around the scar. One common scar that might be present in older patient's is the open gallbladder scar, which is along the rib cage. In that case, if we were to make our normal abdominoplasty incision, remove skin and have a triangle left between that scar and this scar, we could lose some of that skin. Sometimes the Fleur-de-lis abdominoplasty is helpful in removing that old scar and performing a conventional tummy tuck.
