When cataract is not operated in time, it becomes swollen and can lead to a disease called Phacomorphic glaucoma. Cataract surgery in phacomorphic glaucoma is very challenging. This video demonstrates one such case of phacomorphic glaucoma, with IOP controlled with maximal medications but extremely shallow(almost absent) anterior chamber. There is no space to do manipulation in anterior chamber. The most difficult step is to perform capsulorrhexis in such a case. But with patience and proper technique, this can be done.
