He notes that laparoscopic distal and total gastrectomies are oncologically equivalent in early gastric cancers, while robotic surgery offers benefits such as lower morbidity, faster recovery, milder inflammatory response, and improved lymphadenectomy. He also touches on surgery for oligometastasis, showing that operating only on the primary tumor in select cases can slightly improve survival, while metastasectomy often increases morbidity without survival benefit. The session emphasizes the shift toward function-preserving surgeries, such as pylorus-preserving and proximal gastrectomies, particularly in early-stage cancers prevalent in countries like Japan and Korea. These approaches maintain quality of life by reducing dumping syndrome and bile reflux, while preserving nutritional status. Long-term survival rates for these techniques are comparable to standard distal gastrectomy, though some RCT results are still awaited.
